Application Procedure

Units/personnel are to apply for NATO medals while still deployed in the respective operational theatre, through their administrative chain of command. The medal application will be reviewed by SHAPE and an answer provided prior to individuals departing the operational theatre. If approved, medals are to be issued before departure.

If this is not possible, applications should be made electronically, using the standard form below. The responsibility for staffing the requests lies with the National Military Representatives at SHAPE.

The National Military Representatives will forward the requests to the SHAPE NATO Medals Section for approval.

The application form, supporting documentation and a covering letter must be sent electronically, to the respective National Military Representative at SHAPE.

Any request from a Partner nation must be sent to the Military Cooperation Division at SHAPE.
